No romance O Alienista, o Dr. Simão Bacamarte, um médico brilhante e excêntrico, chega à pequena cidade de Itaguaí com o objetivo de construir um hospício e curar todos os que acredita estarem mentalmente doentes. Sua busca por um método universal para tratar a loucura, no entanto, o leva a um caminho perigoso. Bacamarte começa a diagnosticar a insanidade em pessoas cada vez mais 'normais', incluindo membros proeminentes da comunidade, chegando a declarar a cidade inteira como insana. Com o passar do tempo, o hospício se torna um centro de isolamento e controle, gerando revolta entre os cidadãos e colocando em xeque a sanidade do próprio Dr. Bacamarte.
